Day Habilitation Supported Community Connections

We support individuals in building life skills, social connections, community participation, and greater independence through structured day habilitation and community-based activities.

Individual Residential Services and Support

Our Individual Residential Services and Support help individuals receive the support they need in a safe, stable, and person-centered living environment.

Supported Living Services

Supported Living Services help adults with intellectual or developmental disabilities remain in their homes and communities while receiving individualized support based on their needs and goals. Colorado’s SLS waiver is designed to support adults with IDD so they can remain in home and community settings while promoting choice and individualized planning.

CES Waiver Support

We provide support for individuals and families receiving services through the Children’s Extensive Support waiver, helping children with developmental disabilities access services that support safety, stability, and quality of life.

Mentorship

Our mentorship services help individuals develop confidence, life skills, social skills, independence, and meaningful community engagement.

Non-Medical Transportation

Our Non-Medical Transportation support helps eligible members access non-medical community services and supports identified in their care plan, such as community activities, adult day programs, shopping, and community integration activities.
